Referrals

We request referrals to ensure appropriate evaluation and continuity of care. 

Referrals may be sent via fax, email, or electronically through the Epic electronic health record (EHR) system.

Appointments will be scheduled once all relevant clinical notes, lab work, and imaging studies are received.
