Connecting Diabetes Devices

Please follow the instructions below for connecting your diabetes device to our clinic.

Dexcom

  1. Open the Dexcom sensor app on your smartphone

  2. Tap the Connections Tab

  3. Tap Clarity Clinic and tap “Continue”

  4. Enter the Clarity Clinic Code "thriveendo" when prompted.

  5. Confirm clinic details and tap “Done” when finished.

Instructions for connecting Dexcom to endocrinology practice in Ann Arbor
Freestyle Libre

  1. Open the Menu: Tap the three lines (≡) icon in the top-left corner of your FreeStyle LibreLink or Libre app.

  2. Tap Connected Apps

  3. Choose LibreView

  4. Tap Connect to a Practice and enter the Practice ID "thriveendo"

  5. Once you tap Next and Connect, your practice name should appear under Linked Practices
