We are in-network with insurance plans below.

Many insurance plan names are similar, so you can search for "Xin He" on your insurance plan's online provider search tool to see if we are in-network.

If you wish to use insurance, please send your full name, date of birth and a photo of your insurance card to admin@thrive-endo.com.​

Please be informed of any copays, coinsurance, and deductibles, explained here. (Note that deductibles can be $1000+, an amount you pay out of pocket before your insurance begins covering services.)

Out-of-Network or No Insurance

If our practice is out-of-network with your insurance plan, we are still happy to see you and help you reach your health goals.

​

If your insurance offers out-of-network reimbursement, you would provide full payment to the practice at the time of your visit, and we will provide you with a super bill (receipt) of the visit that you can submit to your insurer for reimbursement.

Please contact your insurer to confirm out-of-network reimbursement coverage.
